1. Program Description

By opting in, you agree to receive SMS text messages from Back 2 Health Chiropractic Clinic, which may include appointment reminders, confirmations, office notifications, and occasional and limited promotional or marketing messages. Your mobile number will only be used for SMS communications as outlined in the SMS Privacy Policy.

2. Opt-In Method

You opt in to receive text messages by completing and signing a paper SMS consent form at the chiropractic office. SMS messaging is manually enabled by office staff only after the signed form is received. Consent is not a condition of receiving care or services.

Message frequency may vary based on your appointments and interactions with the office.

Message and data rates may apply depending on your mobile carrier and plan.

5. Opt-Out Instructions

After opting out, you will receive a confirmation message and will no longer receive SMS messages from this number.
